Weighing the Positives

Image: Mehau Kulyk/Photo Researchers, Inc.

It seems like every few months a new study points out the inefficacy of yet another wide-scale cancer screening. In 2009 the U.S. Preventive Services Task Force sug?gested that many women undergo mam?mograms later and less frequently than had been recommended before because there seems to be little, if any, extra benefit from annual tests. This same group recently issued an even more pointed statement about the prostate-specific antigen test for prostate cancer: it blights many lives but overall doesn?t save them.

More recently, researchers at the Dartmouth Institute for Health Policy and Clinical Practice announced that just because a mammogram (almost 40 million are taken every year in the U.S.) detects a cancer does not mean it saves a life. They found that of the estimated 138,000 breast cancers detected annually, the test did not help the vast majority of the 120,000 to 134,000 women afflicted. The cancers either were so slow-growing they did not pose a problem, or would have been treated successfully if later discovered clinically, or else were so aggressive that little could be done about them. Chest x-rays for lung cancer and Pap tests for cer?vical cancer have come under simi??lar criticism.

Individual cases dictate what tests and treatment are best, of course, but one factor underlying all these tests is a bit of numerical wisdom that, though well known to mathematicians, bears repeating: when one is looking for something relatively rare (not just cancer but even for, say, terrorists), a positive result is very often false. Either the ?detected? life-threatening cancer is not there, or it is of a sort that will not kill you.

Rather than looking at the numbers for the prevalence of the above cancers and at the sensitivity and specificity of each of the tests mentioned, consider for illustration cancer X, which, let us assume, afflicts 0.4 percent of the peo?ple in a given population (two out of 500)
at a certain time. Let us further assume that if you have this cancer, there is a 99.5 percent chance you will test positive. On the other hand, if you do not, we will assume a 1 percent chance you will test positive. We can plug these numbers into Bayes? theorem, an im?portant result from probability theory, and get some insight, but working directly through the arithmetic is both more illustrative and fun.

Con?sider that tests for this cancer are administered to one million people. Because the prevalence is two out of 500, approximately 4,000 (1,000,000 x 2/500) people will have it. By assump?tion, 99.5 percent of these 4,000 people will test positive. That is 3,980 (4,000 x 0.995) positive tests. But 996,000 (1,000,000 ? 4,000) of the people tested will be healthy. Yet by assumption, 1 percent of these 996,000 people will also test positive. That is, there will be about 9,960 (996,000 x 0.01) false positive tests. Thus, of the 13,940 positive tests (3,980 + 9,960), only 3,980/13,940, or 28.6 percent, will be true positives.

If the 9,960 healthy people are sub?jected to harmful treatments ranging from surgery to chemotherapy to radi?ation, the net benefit of the tests might very well be negative.

The numbers will vary with different cancers and tests, but this kind of trade-off will always arise in that nebulous region between psychology and mathe?matics. A life saved because of a test, though not that common, is a much more psychologically available outcome than the many substantial, yet relatively invisible, ill effects to which the test often leads.

Meat-eating panda caught on camera

A camera at a Chinese nature reserve has spied a wild panda eating meat.

Pandas spend most of their days eating bamboo.

Staff at the Wanglong Nature Reserve in southwest Sichuan province set up the camera after noticing dead animals with chew marks.

In the footage taken on Nov. 9 by an infrared camera, the giant panda is seen eating a dead gnu. It was not known if the panda had killed the animals.

The Pingwu County forestry bureau says the panda appears to be healthy and strong.

Conservation group WWF says only about 1 percent of a panda's diet is meat or plants that aren't bamboo.

China is set to launch its once-a-decade panda census, state media reported, as it tries to determine how many of the endangered animals live in the wild amid efforts to boost numbers.

How Animals Stay Warm with Blubber

blubber warmth Can landlubbers use blubber?: Discover one of the tricks some animals have evolved to keep warm in chilly waters. Image: George Retseck

Key concepts
Temperature
Heat transfer
Adaptation
Insulation
Fatty tissue

Introduction
Have you ever wondered how whales and other marine mammals survive and keep warm in the cold oceans? Warm-blooded mammals can live in these chilly conditions because their bodies have some cool warmth-saving adaptations, thanks to generations of natural selection.

In other words, to pass on characteristics (via their genes), the predecessors of modern marine mammals had to overcome different challenges to reproduce, and their descendants received the genes that allowed for their survival. This kind of change in organisms over time is what fuels evolution. An important adaptation for marine mammals is blubber, a thick, insulating layer of fat beneath the skin that helps to keep body warmth in and the cold of the air or water out. Will a layer of fake blubber?in the form of shortening?help you keep from getting cold?

Background
Mammals that have evolved to live in cold waters, such as whales, seals, sea lions and polar bears, commonly have a layer of blubber. Whether they are living in cold waters near the North Pole or around Antarctica or are visiting the deep ocean, these animals' blubber is vital to their survival. During the winter, the air in the Arctic (the northernmost part of the world) is often below ?40 degrees Celsius (?40 degrees Fahrenheit). Antarctica, the coldest place in the world, can be below ?60 degrees C (?76 degrees F). Depending on the species, whales dive more than 400 or 500 meters (about one fourth of a mile) deep in the ocean, where the water can be colder than 12 degrees C (54 degrees F).

Blubber helps these marine mammals from getting too cold. (Cold-blooded marine animals, such as fish, sharks or crabs, do not need to stay warm and can let their body temperatures get closer to that of the water. Thus, they do not need to have this extra insulation.) Blubber is a thick layer of fat (adipose) tissue. Animals store extra digested food in the form of adipose tissue, which contains molecules called lipids. Adipose tissue has a relatively low thermal conductivity, which means that it does not transfer heat as well as other tissues and materials?such as muscle or skin. That way, it helps to insulate an animal's body.

Materials
??? ?Two bowls
??? ?Cold water
??? ?Warm water
??? ?Ice cubes
??? ?Shortening (such as Crisco)
??? ?Paper towels
??? ?Stopwatch
??? ?Thermometer
??? ?A partner

Preparation
??? ?Put an equal number of ice cubes into each bowl without filling either bowl too full. Add cold water to each bowl.
??? ?Measure the temperature of the water in each bowl with a thermometer. They should be the same temperature. When the temperature levels off (which should happen quickly), the water is ready for the test.

Procedure
??? ?Cover your pointer finger on one hand with a thick layer of shortening, covering the entire area that will be submerged in the water. Leave your other pointer finger clean and bare.
??? ?Have your partner prepare the stopwatch. When he or she is ready, put the pointer finger of each hand into one of the bowls of ice water and have your partner start timing you. As soon as your finger feels too cold to keep it in the water any longer, take it out. How long did you leave each finger in the bowl?
??? ?Let your fingers warm up and return to their normal color. If any shortening came off of the covered finger, reapply it.
??? ?Have your partner help you pour the cold water down the sink and refill the two bowls with warm water (make sure it is warm but not hot enough to burn the skin).
??? ?Measure the temperature of the water in each bowl with a thermometer. They should be about the same. In the warm water, do you think you'll see the same result?
??? ?Have your partner time how long you can leave each finger in the bowls of warm water. How long did you leave each finger in the bowl? Was the time difference between the two fingers larger or smaller than when you put your fingers in the ice-cold water?? ?
??? ?Extra: How consistent are your results? You can repeat this activity two or three times, recording the temperature of the different waters tested and the time each finger was in the water. Then make a graph out of your results. In which environment did the shortening "adaptation" consistently give an advantage?

Italian antri-trust body fines Apple for misleading customers on product guarantees

Apple Italia has been fined a total of 900,000 euros for failing to inform its customers that products purchased were already covered by two years of free assistance under national law. It appears that Apple employees encouraged the extra purchase of its own AppleCare plans. With the European court's continued investigation of possible e-book price-fixing and ongoing legal jousting with Samsung unlikely to end any time soon, it looks like Apple will be busy in the courts next year too.

Del. judge explains reasoning in Dodgers case

By JESSICA GRESKO

updated 12:38 p.m. ET Dec. 27, 2011

WASHINGTON - A U.S. District Judge in Delaware says a bankruptcy judge erred in freeing the Los Angeles Dodgers from provisions of its current Fox Sports TV contract.

U.S. District Judge Leonard Stark released a 33-page ruling Tuesday explaining his reasoning after he issued an order last Friday to halt the Dodgers' plans to sell the media rights to future games as part of the team's impending sale.

Fox currently has the rights to produce, record and telecast Dodgers games through the 2013 season. The contract gives Fox certain rights in negotiating an extension, including the right to talk exclusively with the team in advance of competing offers. But a judge handling the team's bankruptcy proceedings ruled the "no-shop" provisions were unenforceable in bankruptcy.

Fox has appealed that decision, which hurts its chances of holding on to the broadcasting rights. Lawyers are set to argue the appeal before Stark in January.

Stark said lawyers for Fox have demonstrated a substantial likelihood they will succeed in January's hearing. That's because the bankruptcy judge who freed the Dodgers from the current contract arrangement with Fox improperly relied on a single case in making his decision, Stark said. In addition, Stark wrote that U.S. Bankruptcy Judge Kevin Gross likely made two factual mistakes: finding that the sale of the broadcasting rights was necessary to ensure creditors were paid and finding that the broadcasting rights needed to be sold to maximize the team's value.

The Dodgers sought bankruptcy protection in June after baseball Commissioner Bud Selig rejected a new TV deal with Fox that Dodgers owner Frank McCourt was counting on to keep the franchise solvent. The Dodgers ultimately reached an agreement with the league that calls for a sale of both the team and the media rights. The team must be sold by April 30.

Fox Sports spokesman Chris Bellitti and a Dodgers spokeswoman did not immediately return telephone messages Tuesday requesting comment.

The April 30 sale deadline in the settlement between the Dodgers and MLB coincides with the deadline for McCourt to pay $131 million to his ex-wife, Jamie, as part of their divorce settlement.

Icebreaker reaches stricken Russian fishing vessel (AP)

WELLINGTON, New Zealand ? A South Korean polar research ship on Monday reached a leaking Russian fishing vessel that has been stuck in the frigid waters off Antarctica for the past 10 days, New Zealand officials said.

The Sparta, with 32 crew on board, hit underwater ice on Dec. 16 that tore a 1-foot (30-centimeter) hole in its hull and caused it to list at 13 degrees. Several rescue ships had been hampered by heavy ice in the Ross Sea off the northern Antarctica coast before the icebreaker Araon finally pushed through and reached the Sparta on Monday, New Zealand Rescue Coordination Center spokeswoman Rosalie Neilson said.

The arrival was a relief to the crew, which had been desperately pumping out near-frozen sea water while awaiting rescue. At one point, more than half of those on board were forced onto life rafts.

The crew is made up of 15 Russians, 16 Indonesians and one Ukrainian.

A New Zealand air force cargo plane had previously made two parachute drops of pumps and hull patching gear that had helped keep the single-hulled Sparta from sinking.

Search and rescue coordinator Mike Roberts said the South Korean vessel was alongside Sparta transferring fuel to it to change its trim ? or how it's sitting in the water ? so the bow rises clear of the sea, exposing the damaged area of hull.

Roberts said crew from both ships will attempt to weld a "doubler plate" over the hole ? one external and a second inside. If successful, the repair is expected to make Sparta seaworthy, and should enable it to be escorted by Araon out of the sea ice to open water, he said in a statement. Roberts did not say how long the repair attempt was expected to take.

Weather in the area was calm, which should help the repair operation, he said.

The survival drama on the edge of the Antarctic ice shelf is taking place about 2,200 miles (3,700 kilometers) southeast of New Zealand.

Cubs add two right-handers to bolster bullpen (Reuters)

(Reuters) ? The Chicago Cubs bolstered their bullpen by signing right-handers Manny Corpas and Andy Sonnanstine to one-year contracts, the Major League Baseball team said on Monday.

Sonnanstine, who spent all or parts of the last five seasons with the Tampa Bay Rays, has a career record of 28-31 with one save and a 5.26 earned-run average.

His best season was in 2008 when he was 13-9 in 32 starts helping Tampa win the American League Championship.

Corpas, who missed all of last season after undergoing Tommy John surgery in 2010, has spent his entire career with the Colorado Rockies posting a record of 12-16 with 34 saves and 51 holds in all or part of five Major League seasons.

Santa tracker launches again with smart phone app, volunteers ready

Santa tracker has became a?tradition?for NORAD, which answers 80,000 calls on the whereabouts of Santa. There are several apps available for Apple and Android phones.

The volunteer?Santa?trackers?at the North American Aerospace Defense Command are bracing for tens of thousands of calls and emails when their operations center goes live on Christmas Eve.

The military base has been telling anxious children about?Santa's?whereabouts every year since 1955. That was the year a Colorado Springs newspaper ad invited kids to call?Santa?on a hotline, but the number had a mistake, and dozens of kids wound up talking to the Continental Aerospace Defense Command, NORAD's predecessor.

The officers on duty played along and began sharing reports on?Santa's?progress. It's now a deep-rooted tradition at NORAD, a joint U.S.-Canada command that monitors the North American skies and seas.

Last year,?NORAD?Tracks Santa volunteers answered 80,000 phone calls on?Christmas Eve, said Joyce Creech, project leader.

"It's just so precious to hear the little sigh or breathing on the other end, and you realize how nervous they are," Creech said.

"But we've had really heart-wrenching stories as well," she said. "'Can you ask Santa to heal my brother of cancer?' Or, 'Can I get a new pair of shoes? I don't have any.'"

NORAD's?Santa updates are just about everywhere ? on?Facebook,?Twitter,?YouTube, its?own website?and on television. And this year, there's a new Santa-tracking app for smart phones. The app was downloaded more than 234,000 times from?Android Market?and?iTunes?App Store by mid-December, Creech said.

The?NORAD?Tracks Santa website has had more than 2.2 million unique visitors this year.

But the rows of telephones in the operations center are still the heart of the operation. More than 1,200 volunteers answer calls in shifts, checking big-screen computer monitors indicating Santa's location and passing that along to children, many who seem dumbstruck.

Creech said the rising numbers are probably a reflection of how much people look forward to the season, and how much of a tradition calling?NORAD?has become for many families.

"You can tell that it really brings people joy, and especially kids," she said.

AUTOMOTIVE - DRIVEN: Morgan 3 Wheeler Is Reborn

When the first news reached me in February of a return of the legendary Morgan 3 Wheeler, we were not quite sure how serious to take it. That changed rapidly, and barely six months later I took off in one of the pre-production prototypes from Morgan?s factory in Malvern Link, England.

To understand this rapid development, we have to go back to 2008 when American Pete Larsen launched the Liberty Ace, a car clearly inspired by the great Morgan Aero three-wheelers or trikes of the 1930s. He designed and built the Harley Davidson-powered machine himself, using funding from several clients as well as his thriving side-car business.

The Morgan is powered by a V-twin motorcycle engine, as were the 1930s originals, only now it's a modern S&S with fuel injection. (Photo: Wouter Melissen) One of the people very interested in Larsen?s project was Charles Morgan, the manager director of the 100-year-old British automaker. Morgan travelled to Seattle, Wash., to get a closer look, and he was so impressed with what he saw that he decided to buy Larsen out and bring the project back to the Morgan factory where the first of the original three-wheeled Morgans had been produced.

A lot of work was still needed to get the design up to production-car standards, but Larsen had provided Morgan with a flying start, allowing for a relatively short development period.

The big V-twin mounted between the front wheels rumbles with power, although one of the biggest changes compared with the original design is that it?s no longer a Harley Davidson but a narrow-angle S&S X-Wedge twin. Equipped with fuel injection, dry-sump lubrication and displacing just under two liters, it produces around 120 horsepower and 100 pound-feet of torque.

Bolted directly to the motorcycle lump is a five-speed gearbox straight out of a Mazda Miata MX-5. The sizeable single rear wheel is driven by a belt.

The prototype model was adorned with World War II fighter-style shark graphics. (Photo: Wouter Melissen) The mechanicals are bolted to a simple tubular steel frame that in most respects follows Larsen?s interpretations of the original Morgan design. Suspension is by double wishbones and coil springs at the front, while the rear wheel is fitted between a motorcycle swing arm, which is equipped with twin spring and damper units.

The straightforward package is clothed with an aluminum body complete with the bullet-shaped nose inspired by the 1930s Morgans. Like many of the show cars, the protoype 3 Wheeler was decorated with funky ?shark? graphics that would not have looked out of place on a World War II fighter plane.

In addition to the starter button, the dashboard is fitted with only the bare necessities: a speedometer, tachometer and a row of four toggle switches.

At 6-foot-2, I was a tight squeeze getting into the Morgan, even with the help of a removable steering wheel, but there was just enough room. Thanks to the abundance of torque and power in the 1,100-pound machine, pulling away is easy and spinning the rear tire in first or second takes very little encouragement.

Scott Weiland's 'Winter Wonderland' Frame By Frame

Foster the People and others help break down the Stone Temple Pilots frontman's Christmas video.
By Matt Elias

With Christmas just around the corner, it's safe to assume you're enveloped in holiday tunes — whether inescapably at the mall or blaring from your laptop by choice. Beyond the old standards, there's Michael Bublé's chart-topping Christmas album and even Justin Bieber's first Christmas contribution, Under the Mistletoe, but you might not be aware that Stone Temple Pilots frontman Scott Weiland released his own holiday disc this year.

That's right, and while Weiland's The Most Wonderful Time of the Year dropped back in October, even some of his contemporaries were unaware of his Xmas effort. That gave us the perfect opportunity to share the album's first music video with the bands on the bill at KROQ's annual Almost Acoustic Christmas earlier this month. In true holiday spirit, the artists graciously took us through the kitschy retro-themed clip for "Winter Wonderland," frame by frame.

Initially, everyone was surprised by the video, in part due to Scott's never-before-heard classic croon, but also by his "Mad Men"-inspired, clean-cut new look.

"I like his scarf a lot," Marcus Mumford of Mumford & Sons said.

"He's very handsome," fellow bandmember Winston Marshall added.

While the reactions ran the gamut from "good" to "insane," there were other clues in the video, some believed, that this whole thing might be more of a wink and a nod than a serious musical venture.

"He kinda has a very weird eyebrow thing that I only noticed now for the first time," Grouplove's Ryan Rabin said.

"I can't tell if it's tongue-in-cheek or not," Mark Foster of Foster the People wondered. "You think it's tongue-in-cheek or you think he just lost his mind? 'Cause he definitely had a bout with [in]sanity for a while."

Of course, the sight of a '90s rock icon cozying up to the Yule log left some baffled, but Death Cab for Cutie had their own take on Weiland's "Wonderland."

"I mean, is he more Bing Crosby or is he more Danny Kaye?" DCFC's Chris Walla asked.

"He's got a lotta Bing, he's got a lot of ... I mean there's some Sinatra there too," DCFC bassist Nick Harmer said.

They both agreed that Weiland was fully committed to the act.

"And it's good! It's classic," Walla said.

"Thank you! I mean — yeah! Thanks for making that Scott, seriously," Harmer concluded.

What do you think of Weiland's version of "Winter Wonderland"? Tell us in the comments.

40 Years On, the Triumphs and Challenges of America's 'War on Cancer' (HealthDay)

WEDNESDAY, Dec. 21 (HealthDay News) -- Jack Whelan first knew something was wrong when it got harder and harder to walk from the train station in Boston to the financial district where he worked.

He knew something was terribly wrong when he started getting nose bleeds.

A consultation with an oncologist confirmed Whelan's fears: He had advanced Waldenstrom's macroglobulinemia, a rare form of blood cancer that affects only about 1,500 people in the United States each year.

Forty years ago, Whelan would have had five years to live -- at the outside -- and who knows what his quality of life would have looked like.

But today, five years after his diagnosis and almost 40 years to the day that President Richard Nixon signed the National Cancer Act declaring "war" on cancer, Whelan, 63, is power-walking, raking leaves, shoveling snow and back at work as a marketing executive.

Whelan is just one of the millions of Americans who have benefited from continued advances in cancer research. He has participated in four different clinical trials and is currently taking an experimental drug called LBH589 which, Whelan said, makes him "feel like Popeye the sailor after having spinach."

Just this month, scientists at the Dana-Farber Cancer Institute in Boston, where Whelan is being treated, discovered a single gene mutation present in 90 percent of patients who have this rare type of cancer, raising the hope that an even more targeted treatment will soon be able to attack the disease.

Since Dec. 23, 1971, and the passage in Congress of the National Cancer Act, research has made tremendous progress against what is still one of the world's foremost killers, experts say.

"Back at that time point, cancer essentially was a death sentence," said Dr. Raymond N. DuBois Jr., provost and executive vice president for academic affairs at M.D. Anderson Cancer Center in Houston.

That's no longer the case, however, thanks to advances in early detection, improved therapies and a better understanding of the genetics driving different forms of cancer, he said.

"Forty years ago, fewer than one-third of patients with a diagnosis of cancer lived five years. Almost no children with a diagnosis of the most common form of childhood cancer, acute leukemia, lived [that long]," said Dana-Farber president Dr. Edward Benz Jr. "In 2011, nearly 90 percent of children diagnosed with acute leukemia will be cured and nearly two-thirds of all people diagnosed with cancer will live at least five years."

Since 1991 alone, there's been more than an 18 percent reduction in deaths from cancer, added Dr. Otis Brawley, chief medical officer of the American Cancer Society.

An ounce of prevention

Much of this progress may have started with prevention.

Declines in smoking rates, helped by the landmark U.S. Surgeon General's Report in 1964 linking smoking to cancer, have continued over the decades, preventing countless cases of lung malignancies and other forms of cancer.

Colonoscopies to detect pre-cancerous polyps have not only reduced mortality but prevented many cases of colorectal cancer outright.

The adoption of regular mammography screening for breast cancer is another success story in its own right, as is screening for cervical cancer.

Thanks to, first, the Pap smear (which looks for abnormal cells on the cervix) and now the HPV test (which detects the human papillomavirus that can cause cervical cancer), death rates from cervical cancer in the United States plummeted more than 60 percent between 1955 and 1992, according to the U.S. National Cancer Institute.

No doubt, incidence and mortality from cervical cancer will continue to decline with the advent of another major weapon: newly approved vaccines that prevent infection with the strains of HPV that cause most cases of this type of cancer.

These vaccines (two have been approved by the U.S. Food and Drug Administration) have great potential to reduce head and neck cancers, as well as anal cancer deaths, which can also be caused by HPV, Brawley said.

But advances in detection have been complemented by improvements in treatment, the experts added. These include better surgical techniques. For example, studies suggest that women who have a lumpectomy to conserve their breast along with radiation typically have as good a prognosis as women who undergo a full mastectomy.

Targeted radiation has also made treatment much less onerous for prostate cancer patients, and new chemotherapies often arrive with drastically fewer side effects than in decades past.

The age of "targeted therapies" or "personalized medicine" -- an era ushered in by anti-estrogen breast cancer therapies such as tamoxifen (which debuted in the 1980s) -- is here, Brawley said. Those highly targeted medications were later joined by aromatase inhibitors as well as Herceptin (trastuzumab) to attack a specific form of Her2neu-positive breast cancer.

Scientists are also finding new targets for lung, colorectal and other cancers. For example, studies show that Tarceva (erlotinib) can improve the average survival of patients with non-small cell lung cancer by about two months. That may not sound like much but, in lung cancer, it represents a huge stride.

"Wonder drug" Gleevec, a medicine used to push certain blood cancers into remission, is another targeted-therapy success story. In fact, a colleague of Whelan's was diagnosed with chronic myelogenous leukemia while still in his 20s and subsequently died. Had he been diagnosed a few years later, after the discovery of Gleevec, he would have lived, Whelan believes.

Brawley agrees that "personalized medicine is the future," and he predicts many more advances in this area in the next five years.

Dubois added: "We are doing molecular fingerprinting of each individual tumor and, although we're not using that right now to direct cancer care, the idea is once we have that information we will be able to use it to figure out exactly which treatments a patient needs so they're not being given unnecessary treatment. And the treatment they do get is going to be much more effective on the first round of therapy when it really makes the biggest difference."

Doctors now also know that "multi-modality" therapy -- meaning the combined use of surgery, radiation and drug therapy -- "has given people the best chance for good outcomes for particular kinds of cancer," said Benz.

Progress lacking on some fronts

But while there's been undisputed progress, "it's very incomplete progress," Benz and others acknowledged.

"If you look over the past 40 years, on some fronts we've actually been winning and on some fronts we're losing terribly," said Brawley. "We are our own worst enemy in terms of battling cancer with tobacco control, diet and exercise and getting everybody adequate preventive screening and treatment.

"In excess of 200,000 of the 500,000 lives that will be lost from cancer this year could have been avoided if we simply adopted all the cancer-control technologies that we've learned over the last 40 years," he added.

Although the smoking rate has declined dramatically since publication of the U.S. Surgeon General's 1964 report, it's been stalled at about 20 percent for 10 years now, Brawley said.

There are also lingering disparities in both prevention and treatment by race, socioeconomic status and urban versus rural locations, said Brawley.

Cancer therapies are also becoming increasingly complicated and expensive "at a time when the trend in health care and in support for cancer research is going down," added Benz. "I worry that we're going to see increasing disparities as cancer and personalized medicine becomes more complicated and expensive. It will be harder and harder to offer it to everybody who needs it."

Clinical trials may also become more difficult and expensive to conduct, as scientists recognize more and more subtypes of cancer. That means fewer people fit each particular subtype, Benz said.

Nevertheless, the overall message is a positive one.

"It's been a huge evolution since 1971," said DuBois. "It's just incredible."

Time short for S&P to end 2011 higher (Reuters)

NEW YORK (Reuters) ? With two weeks left in the trading year, the euro-zone debt crisis will remain the primary impediment to pushing the S&P 500 index into positive territory for 2011.

Uncertainty over progress in the region, along with the potential for credit rating downgrades on euro-zone countries, have kept investors on edge and market volatility high.

Even with a fairly busy U.S. economic calendar, which includes a batch of data on the housing market, the final reading on gross domestic product and durable goods orders, markets will focus on developments from Europe.

"What everybody is going to look at is the same thing they've been looking at -- every time a German official opens their mouth, we get crushed," said Paul Mendelsohn, chief investment strategist at Windham Financial Services in Charlotte, Vermont.

"I'm keeping my fingers crossed that Santa Claus is out there. But we've got to see something."

The benchmark S&P 500 index (.SPX)(.INX) is down about 3 percent for the year and would need to climb above 1,257.64 in order to end higher for the year.

A stocks' rally on Friday fizzled, and the market ended with only modest gains after the latest credit warning about possible downgrades of European nations. For last week, the Dow fell 2.7 percent, the S&P 500 lost 2.9 percent and the Nasdaq slid 3.5 percent.

Italy's prime minister urged European policymakers on Friday to beware of dividing the continent in the effort to contain the debt crisis, warning against a "short-term hunger for rigor" in some countries, in a swipe at Germany.

Stocks have been whipsawed as investors weigh the threat from the euro-zone crisis against modest improvement in U.S. economic data and stocks that many regard as cheap.

"There do appear to be some improving economic indicators domestically, but it's hard to see how they win the day if Europe continues to be a big concern. It's not like the valuations are at such bargain-basement prices that it becomes a one-way bet," said Stephen Massocca, managing director at Wedbush Morgan in San Francisco.

As volumes begin to dry up and market moves become more exaggerated during the holiday period, the volatility may help lift the stock market into the plus column.

CHANCE OF RALLY

"Can you see an upside rally? Certainly, because you are going to have some asset managers in the end who are going to try and just push it so the market ends at the very least flat on the year, if not higher," said Ken Polcari, managing director at ICAP Equities in New York.

"If there is going to be a rally at all, it will happen on light volume because there will be fewer and fewer participants. When there is less volume, you do have the ability to have those exaggerated moves, but people will take advantage of that."

Volatility in individual shares could also be affected by corporate earnings pre-announcements. There have been 97 negative earnings pre-announcements issued by S&P 500 corporations for the fourth quarter, compared with 26 positive pre-announcements, resulting in a negative-to-positive ratio of 3.7. That's the highest in 10 years, according to Thomson Reuters data.

Companies that have provided outlooks in recent weeks include DuPont (DD.N), Intel Corp (INTC.O), United Technologies Corp (UTX.N) and Texas Instruments Inc (TXN.N).

Unexpected management shakeups could also be on the horizon and increase the tumult in stocks. Both Cablevision Systems Corp (CVC.N) and The New York Times Co (NYT.N) saw high-level executives suddenly leave their posts.

But stock movements this week will ultimately be dictated by actions taken in Europe, with the light volume exacerbating market swings.

"The only thing that is going to be of any interest is certainly the continuing headlines on Europe, whether or not they come any closer to what looks like a potential agreement," said Polcari.

"You may get a little bit of a push to the 1,250 to 1,270 range, but much beyond that I don't see why it would go any higher unless you get some explosive announcement out of Europe."

Atheist intellectual Christopher Hitchens dead at 62 (Reuters)

WASHINGTON (Reuters) ? British-born journalist and atheist intellectual Christopher Hitchens, who made the United States his home and backed the 2003 U.S. invasion of Iraq, died on Thursday at the age of 62.

Hitchens died in Houston of pneumonia, a complication of cancer of the esophagus, Vanity Fair magazine said.

"Christopher Hitchens - the incomparable critic, masterful rhetorician, fiery wit, and fearless bon vivant - died today at the age of 62," Vanity Fair said.

A heavy smoker and drinker, Hitchens cut short a book tour for his memoir "Hitch 22" last year to undergo chemotherapy after being diagnosed with cancer.

As a journalist, war correspondent and literary critic, Hitchens carved out a reputation for barbed repartee, scathing critiques of public figures and a fierce intelligence.

In his 2007 book "God Is Not Great: How Religion Poisons Everything," Hitchens took on major religions with his trenchant atheism. He argued that religion was the source of all tyranny and that many of the world's evils have been done in the name of religion.

The son of a British naval officer, Hitchens studied at Oxford University and worked as literary critic for the New Statesman magazine in London before moving to New York to work as a journalist in 1981. He settled in Washington the following year, initially as correspondent for the left-wing magazine The Nation. He retained his British citizenship when he became an American citizen in 2007.

Hitchens was not one to mince words. In his book on Bill Clinton "No one left to lie to", he called the former U.S. president a "rapist" and a "con man." He once referred to Mother Teresa of Calcutta as a "fanatical Albanian dwarf."

The author of 25 books - including works on Thomas Jefferson, Thomas Paine and George Orwell - and countless articles and columns, Hitchens never lost his biting humor.

"CANCER ELITE"

"I'm a member of a cancer elite. I rather look down on people with lesser cancers," Hitchens said in an interview with CBS "60 Minutes" aired on March 6, 2011.

In a 2010 interview with Reuters, Hitchens dismissed criticism that he moved from left to right and helped former U.S. President George W. Bush sell the 2003 war with Iraq to the American public with what turned out to be bad intelligence about weapons of mass destruction.

"Saddam was an enemy of the civilized world and he should have been taken out a long time before," Hitchens said of Iraqi leader Saddam Hussein. "I have no regrets about that at all."

The 2001 attacks on the United States by Islamic fundamentalists in hijacked passenger planes made Hitchens ever more critical of the role of religion in the world, and led him to appreciate the merits of American democracy.

"I am absolutely convinced that the main source of hatred in the world is religion, and organized religion," he wrote.

Hitchens is survived by his wife, Carol Blue; their daughter, Antonia; and his children from a previous marriage, Alexander and Sophia, Vanity Fair said.

In his last essay on www.vanityfair.com, dated "January 2012," Hitchens said his illness made him question the saying attributed to German philosopher Friedrich Nietzsche that "Whatever doesn't kill me makes me stronger."

A painkiller injection just before typing the article titled "Trial of the Will," Hitchens wrote, caused "numbness in the extremities, filling me with the not irrational fear that I shall lose the ability to write. Without that ability, I feel sure in advance, my 'will to live' would be hugely attenuated."

Philippines OKs Xmas truce with communist rebels (AP)

MANILA, Philippines ? Philippine President Benigno Aquino III has approved a cease-fire with communist rebels over the Christmas and New Year holidays in a traditional good will gesture.

Government troops were ordered Thursday to refrain from offensive operations and return fire only in self-defense.

The rebels usually reciprocate with their own cease-fire. They've been fighting for a Marxist state for decades despite their dwindling numbers.

The rebel New People's Army recently stepped up attacks on foreign mining companies, accusing them of exploiting resources and local workers. The government accuses the rebels of extorting money from businesses.

Marathon peace talks brokered by Norway have stalled over rebel demands for the release of jailed guerrillas.

Police find fifth victim in Belgium grenade attack

By msnbc.com and news services

LIEGE, Belgium - Investigators have found the body of a woman in a shed belonging to?the gunman who killed four people and wounded 125 in eastern Belgium on Tuesday.

The body was found following an overnight search of the building, which police said shooter Nordine Amrani had used to grow cannabis.

People flee the grenade attack in the heart of Liege, Belgium on December 13.

The 33-year-old brought carnage to the eastern city of Liege by hurling grenades and spraying bullets into crowds of Christmas shoppers.

Two boys aged 15 and 17 were killed, along with a 75-year-old woman and a toddler of just 17 months, whom hospital doctors fought for hours to save.

It was still unclear what drove Amrani to carry out the attack, and investigators were trying to build a profile of the man.

He was previously jailed for possession of arms and drugs offenses.

On the morning of the killings he had been called in for questioning by police in a sexual abuse case, the Associated Press reported.

A body, suspected to be gunman Nordine Amrani who opened fire children and Christmas shoppers in Leige, Belgium.

Police said the body of his fifth victim was a 45-year-old woman who worked as a cleaner for one of his neighbors.

Prosecutor Cedric Visart de Bocarme told Belgian La Premiere radio station: "A search last night revealed in a warehouse used by the attacker, notably to grow cannabis, the body of a woman killed by the attacker before he went to the Place Saint Lambert.

"The enormous concern we have is how it was possible for someone seemingly sensible and normal to do this. It was a delinquent, someone who had difficulties throughout their life."

Previous conviction
Amrani was freed from jail about a year ago following a 2008 conviction for illegal possession of arms and for growing a huge field of cannabis. A report in Britain's Daily Telegraph?said police found 10 firearms and 9,500 gun parts along with 2,800 cannabis plants, but a prison official granted Amrani early release last year.

The report said he was a weapons aficionado and was said to be able to dismantle, repair and put together a number of different weapons but was never linked to any terrorist act or network.

"He has no history of terrorist acts," prosecutor Daniele Reynders told a news conference on Tuesday.

Instead of reporting to the police station as requested on Tuesday, he took a light automatic rifle, a handgun and several grenades and went onto the roof of a bakery in the central Saint-Lambert square before hurling the weapons into the crowd.

How exactly he died was not immediately clear, with some witnesses claiming he turned his revolver on himself while others said one of his grenades appeared to explode prematurely.

Liege's mayor, Willy Demeyer, said the two teenage victims had been taking school exams nearby just before the attack.

At Amrani's most recent address, an apartment block near the scene of the attack, Johan Buron said he had been astonished to learn what his neighbor had done.

"He was calm, every time I met him in the corridor he was very friendly and said 'Hi'," he told Reuters. "If my memory serves me right, he was a welder."

Random killings are rare in Belgium. Most recently, in January 2009, a man stabbed two infants and a woman to death and injured 13 at a nursery in the town of Dendermonde.

Reuters, The Associated Press and msnbc.com staff contributed to this report.

APNewsBreak: Man charged in attack on Simmons site (AP)

LOS ANGELES ? A Connecticut man affiliated with the Anonymous hacking group was arrested Tuesday on federal charges for an attack on a website belonging to Kiss bassist Gene Simmons, authorities said.

Kevin Poe, of Manchester, Conn., made his initial appearance Tuesday in federal court in Hartford, Conn., and was released on $10,000 bond. An email message left for deputy federal public defender Deirdre Murray was not immediately returned.

Poe was indicted in Los Angeles on two counts: conspiracy and unauthorized impairment of a protected computer. If convicted of both, he faces up to 15 years in prison.

Poe and others linked to Anonymous conducted a "denial of service" attack over a five-day period last October against Simmons' computer systems, sending tens of thousands of electronic requests designed to overload the server and render www.GeneSimmons.com useless.

Poe used a computer program favored by Anonymous that sends extremely large requests over a network in an attempt to overwhelm a target computer, according to the indictment.

Authorities didn't say why Simmons was targeted, but he made comments against copyright infringement at a convention in Cannes, France, days before the cyber attack.

No other arrests have been made.

Representatives for Simmons couldn't immediately comment about Poe's arrest because they weren't aware of it.

Corzine says gave no order to move MF Global funds (Reuters)

WASHINGTON (Reuters) ? Former MF Global chief Jon Corzine said on Tuesday he felt a need to clarify his prior statements, definitively telling a new panel of lawmakers that he never gave instructions that could have been misinterpreted as permission to misuse customer funds.

The Senate Agriculture Committee was holding on Tuesday the second hearing in under a week to feature Corzine, who has said he does not know the location of hundreds of millions of dollars of customer money missing from the futures brokerage.

Corzine told lawmakers last week that he "never intended" to break rules about keeping customer funds segregated from firm money, but said that an employee may have misinterpreted instructions to try to save the firm.

On Tuesday, Corzine said, "I want to be clear - I never gave any instructions to misuse customer funds, I never intended anyone at MF Global to misuse customer funds."

The futures brokerage filed for bankruptcy on October 31, after it was forced to reveal it bet $6.3 billion on European sovereign debt. That disclosure caused ratings downgrades and saw nervous investors and customers begin to flee the firm.

Corzine resigned as chief executive days after the bankruptcy filing.

The search for the missing customer funds has sent reverberations through the farm belt and trading floors, and has attracted the attention of the FBI and federal prosecutors. They are trying to determine whether customer funds were diverted to firm accounts, a major violation of industry rules.

In the meantime, thousands of customers have had their money frozen, although some has been returned as the trustee in the bankruptcy sorts through MF Global's records.

Corzine reiterated on Tuesday that he does not know where the money is, but he was willing to speculate on why funds are not yet accounted for.

He said in the final days of MF Global, he directed the sale of $1.3 billion in commercial paper, hundreds of millions of dollars in MF Global proprietary assets, and $4.5 billion in government agency bonds.

"As I sit here, I didn't, do not know whether all of these and many other transactions were properly recorded and effectuated, or whether banks and other counterparties involved in such transactions properly credited the right accounts for these sales," Corzine told lawmakers.

He also revealed that JPMorgan Chase contacted him and other MF Global employees on October 28 about "certain overdrafts and whether funds had been transferred in compliance with (Commodity Futures Trading Commission) rules."

Corzine said he had no personal knowledge about the issue, so he asked senior people in MF Global's back office in Chicago and in the firm's legal department to become directly involved.

"So even in the midst of the chaotic last days of business, I had confidence in our people and systems," Corzine said.

'DARK AGES'

Two top-ranking company executives, Chief Operating Officer Bradley Abelow and Chief Financial Officer Henri Steenkamp, appeared on the same panel as Corzine. They also said they lack answers about the money.

Senators were frustrated by the testimony, frequently asking the executives and Corzine "Where's the money?" and "What happened?"

"It's now been a month a half since the firm collapsed and customer money is still nowhere to be found," Senate Agriculture Committee Chairwoman Debbie Stabenow said.

"This isn't the Dark Ages. MF Global didn't keep their books with feather quills and dusty ledgers. The rules about keeping customer money segregated are pretty straightforward."

Abelow and Steenkamp said they do not know what happened.

"I unfortunately have limited knowledge of the specific movement of funds at the U.S. broker-dealer subsidiary, MF Global Inc, during the last two or three hectic business days prior to the bankruptcy filing," Steenkamp said in his testimony.

The court-appointed trustee has estimated the shortfall of customer money at $1.2 billion, but other regulators have disputed that amount as too high.

'RIPPLE EFFECT'

Farmers who became collateral damage from the collapse of MF Global got a chance to air their frustrations, telling lawmakers on Tuesday that their confidence in the markets has been shaken.

Dean Tofteland, a corn and soybean farmer from Minnesota, said when he heard news that MF Global was having problems, he talked to his broker, who told him, "No customer has ever lost a penny in customer segregated accounts."

But three days later his $253,000 account was frozen and he could not adjust his short positions.

Tofteland's positions were transferred to a new broker with only 15 percent of the required collateral, and he was forced to liquidate the hedges, he said. Since then, prices dropped and he lost another $100,000 without having the hedge.

Tofteland said he has not returned to the futures market.

"What they call 'unlawful comingling' on Wall Street is called 'stealing' back on Main Street," he told lawmakers.

C.J. Blew, a Kansas farmer and rancher, said the agriculture sector needs answers about what happened to the customer money.

"This has a ripple effect, I think, throughout the industry. You got farmers double margining. You got people with stress on their lending situations already so it's important that we get to the bottom of it," Blew said.
